Transferred the loading of the arrays for the booking forms into a own separate
...
method.
Impemented the booking.edit view and controller method.
Fixed the bug in the bookings.index view that only months with cashflow are
listed.

1. Added csrf field.
2. Added dropdown menue for the title.
3. Added an input for the date of birth.
4. Fixed the appearance of the member state edit section.
